United Healthcare and other Banner Aetna plans: I am not in-network at this location. Your insurance company may grant a waiver or gap exception if you request it BEFORE your visit. Please call your insurance company and ask for approval to see me out-of-network!

Out-of-network superbill option: pay upfront, and superbills can be provided upon request so you may submit a claim to your insurance for reimbursement.

Out-of-network billing available: We can bill your out-of-network insurance company for a $15 processing fee plus $50 deposit. If your insurance pays at the full fee or above, your deposit will be refunded. If your insurance does not pay the full fee, you will be invoiced for the balance of the visit. Requires credit card on file. Your insurance may require a Gap Exception or Single Case Agreement – please call your insurance PRIOR to your appointment to get this authorization. If you do not call for this authorization, your claim could be denied and you would be responsible for the full fee.
