Home visits: insurance billing only available for Aetna and Blue Cross Blue Shield private plans. All other insurance plans are pre-pay for superbill and you submit your own reimbursement request. Home visits are provided within 15 miles of 85206 – $25 fee for addresses beyond this radius in 15-mile increments.
